Compensation

Per diem pay: $Hourly pay up to $160-180/hour

Duties

• Perform in-home physical therapy evaluations and treatments
• Develop and adjust individualized plans of care
• Treat Part A ped cases
• Complete timely and accurate EMR documentation
• Reassess progress and update treatment plans
• Educate patients and caregivers on exercises and safety
• Coordinate care with physicians and healthcare team
• Manage daily schedule and optimize patient routing
• Maintain productivity and quality of care standards

Requirements

• Valid licensure or certification as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist in ALL NYC BOROUGHS.
• Proven experience in home healthcare, NEW GRADS ARE WELCOMED
• Familiarity with EMR systems for documentation and patient tracking
• Experience with discharge planning, acute care procedures, and working within multidisciplinary teams
• Excellent communication skills to educate patients and coordinate with healthcare professionals effectively
