- Conduct comprehensive patient assessments and develop personalized, evidence-based treatment plans for patients in their homes.
- Provide specialized physiotherapy interventions (manual therapy, therapeutic exercises, patient education, etc.).
- Monitor patient progress, adjust treatments, and ensure timely and safe discharge planning.
- Supervise, mentor, and train other Homecare Physiotherapists and assistants within the assigned zone.
- Manage the caseload allocation and scheduling for the zone to ensure optimal coverage and service delivery.
- Ensure the quality and standards of physiotherapy services across the zone comply with organizational and regulatory guidelines.
- Conduct audits and quality checks on patient documentation and clinical outcomes.
- Liaise with the central management team, other zonal heads, and interdisciplinary teams (doctors, nurses, etc.) for care coordination.
- Manage the inventory and maintenance of physiotherapy equipment for the zone.
- Handle escalated patient/caregiver feedback or complex cases within the zone.
