Homeless Outreach Case Manager

Position Overview: Reporting to the Director of Case Management Services, the Homeless Outreach Case Manager is responsible for assisting clients who are living on the streets (or in other places not meant for human habitation) to end their homelessness and meet their basic needs, with a focus on serving those who are not being served in traditional drop-in or shelter environments for any reason. The Homeless Outreach Case Manager assist individuals in Gentilly and New Orleans East with identifying and overcoming barriers to housing access and will also be responsible for identifying immediate needs, educating individuals on their options, navigating individuals through public and nonprofit systems, and accessing critical supports such as housing, healthcare, documentation, and income resources for individuals.

Key Responsibilities

Essential Duties

  • Serve as a guide and advocate for people living on the streets or in places not meant for human habitation.
  • Proactively canvas an assigned area by foot or bicycle to build consistent, low-barrier relationships with unhoused individuals
  • Offer immediate support, referrals, and problem-solving for unmet needs
  • Work collaboratively with clients to navigate systems (ex: housing intake, health insurance, ID acquisition, public benefits)
  • Advocate for clients in their navigation of services and proactively move the client cases forward (writing/calling service providers; accompanying clients to court, hospital, clinics, etc.)
  • Maintain a rolling navigation caseload of up to 20 individuals with high vulnerability with whom the outreach case manager will have a minimum of 35 monthly contacts (overall). The caseload will be composed of referrals from partner agencies as well as from the case navigator's canvassing efforts
  • Develop short-term navigation plans with each client based on their goals, needs, and service readiness

Administrative Duties and Reporting

  • Document all client-based encounters within 24-48 hours of an event in the appropriate electronic record system (Salesforce).
  • Participate in supervisory meetings, weekly team meetings, and agency-related meetings.
  • Act in accordance with the agency’s guidelines and policies.
  • Display professional conduct and behaviors in-office and off-site.
  • Collaborate with Director of Case Management Services for reporting and supervisory needs with open communication and responsiveness.

Qualifications

  • Bachelor’s degree in social work, psychology, human services, or a related field. Master's degree preferred.
  • Education qualification and 1+ year of case management/outreach experience serving similar populations or 3+ years of employed case management/outreach experience with similar populations. Individuals with lived experiences encouraged to apply.
  • Must have a valid Louisiana Driver’s license with a good standing driver’s record.
  • Ability to manage multiple tasks, prioritize effectively, and meet deadlines in a fast-paced environment.
  • Strong written and verbal communication skills, including the ability to report on outcomes and engage with staff and stakeholders.
  • Knowledge of case management software (electronic records) and tools.
  • Proficient in Microsoft Office
  • Strong organization skills
  • Strong ethical decision-making, and a willingness to bring situations to supervision for review.
  • De-escalation/crisis response skills/experience
  • Organizational skills (client file, case, and calendar maintenance)
  • Experience with navigating bureaucracies including Food Stamps, Medicaid, Social Security, and local health/housing providers
  • A willingness to collaborate openly and often with colleagues (internal and external to agency)

Location: 1340 Poydras Street, Suite 1750, New Orleans LA 70112. Ability to travel within the New Orleans area as needed to meet clients or attend meetings. Navigators operate primarily in the field — conducting outreach, providing navigation support, and meeting clients in the settings where they live and spend time. Work occurs in the Gentilly and New Orleans East during daylight hours only. Fieldwork may involve walking long distances, sitting on the ground, or carrying a backpack.

Job Type: Full-time (35 hours per week), Monday through Friday (with alternating Fridays off), 7:30am to 3:45pm.
