Job Title: Homeless Outreach Case Manager
Reports to: Crisis Response Supervisor
Education: Bachelors in Human Service, or equivalent experience
Location: 1081 Broadway, Buffalo, New York
Other Requirement: A valid NYS Drivers License and a reliable insured vehicle
The Homeless Outreach Case Manager is a field position that has the responsibility of engaging with street homeless and chronically homeless individuals in the City of Buffalo and assist them with meeting their basic needs. The case manager will also link clients with necessary services, and identify service providers already working with clients. The case manager will work collaboratively with various entities, ensuring that the needs of identified clientele are met and efficient follow up care is conducted in a timely manner.
Other responsibilities include:
- Perform street outreach in the community, as well as at various service locations clients are known to congregate
- Carry a case load of street homeless individuals and provide case management
- Maintain regular contact with street homeless clients
- Maintain client confidentiality
- Respond to outreach referrals in a timely manner, while making earnest effort to locate referred individuals
- Advocate for homeless clients when they encounter barriers, such as navigating clients through the ECDSS system and other government bodies to secure entitlement benefits
- Establish and maintain positive, productive working relationships with mental health programs, shelter programs, police (and other local officials), and providers of services and resources to homeless clients
- Provide service linkages for client to any resources the client identifies as a need (Mental Health, Physical Health/Primary Care, Substance use services)
- Provide 30 days of aftercare once a client obtains housing
- Input all data, service linkages, case notes and other assessments into HMIS
- Ensure quality of data input into HMIS is at 97% (HUD requires 95%)
- Attend Homeless Outreach Committee meetings, team meetings, case conferences, trainings and community meetings as needed
- Participate in Friday early morning/ Tuesday night outreach to the homeless
- Take an active role in the Hope Center activities and service provision to Hope Center clients
- Other duties as assigned
