Job Overview
One Health Hospice, Inc. is seeking compassionate, dependable, and patient-centered Hospice Aides (CHHA) to join our growing hospice team serving patients throughout Contra Costa County, Alameda County, and San Joaquin County.
The Hospice Aide provides personal care and supportive services to terminally ill patients in their homes, assisted living facilities, board and care homes, and skilled nursing facilities under the supervision of a Registered Nurse. We are looking for team members who are compassionate, reliable, respectful, and passionate about providing comfort-focused end-of-life care.
Responsibilities
- Provide personal care assistance including bathing, grooming, oral care, toileting, dressing, and skin care
- Assist patients with transfers, ambulation, repositioning, and range of motion exercises
- Observe and report changes in patient condition to supervising nurse
- Maintain accurate visit documentation and timely charting
- Provide emotional support and compassionate presence to patients and families
- Follow infection control and safety protocols
- Assist with light housekeeping related to patient care
- Support patients with dignity, comfort, and respect during end-of-life care
Qualifications
- Current California Certified Home Health Aide (CHHA) certification required
- Current BLS/CPR certification preferred
- Hospice or home health experience preferred, but willing to train the right candidate
- Valid California Driver’s License, reliable transportation, and auto insurance required
- Ability to travel throughout assigned territories
- Strong communication and interpersonal skills

Pay: $55,000.00 - $65,000.00 per year
