Hospice Case Manager

Join our exceptional & compassionate team is offering a premier role as a Hospice Case Manager, where you will play a vital role in providing patient-centered care to individuals facing end of life illnesses. This dynamic position involves coordinating comprehensive care plans, supporting patients and their families through difficult transitions, and ensuring the highest quality of comfort and dignity at every stage. As a key member of our interdisciplinary team, you will leverage your clinical expertise and empathetic approach to deliver exceptional hospice services that prioritize patient wishes and promote quality of life.

Responsibilities

  • Conduct thorough patient assessments to determine eligibility, needs, and care goals, utilizing your expertise in medical terminology, physiology, and symptom management.
  • Develop and implement individualized care plans that incorporate pain management, symptom control, psychosocial support, and spiritual care.
  • Coordinate with healthcare providers across hospital medicine, outpatient clinics, post-acute facilities, and community resources to ensure seamless continuity of care.
  • Provide education and emotional support to patients and families navigating complex health decisions while maintaining compliance with HIPAA regulations.
  • Monitor patient progress through regular visits—whether inpatient or outpatient—and adjust care strategies accordingly to optimize comfort and dignity.
  • Manage discharge planning and facilitate transitions between hospital settings, skilled nursing facilities, or home environments with attention to medical records accuracy and documentation.
  • Engage in utilization management and diagnostic evaluation processes to ensure appropriate resource utilization aligned with patient needs.

Qualifications

  • Proven experience in comfort care not limited to experience in critical care environments such as ICU, emergency room.
  • Strong background in hospice medicine, with familiarity in inpatient and outpatient services.
  • Knowledge of Geriatrics, behavioral health issues, and hospice experience is mandatory to apply.
  • Demonstrated ability in patient assessment, discharge planning, intake procedures, and case management within hospice settings.
  • Excellent understanding of medical terminology, vital signs monitoring, diagnostic evaluation techniques, and physiology fundamentals.
  • Experience with managed care processes, utilization management protocols, medical records documentation, and HIPAA compliance standards.

Pay: $55.00 - $250.00 per hour
