The RN Case Manager is responsible for providing nursing expertise, managing all services needed as determined by the Plan of Care, and assuring delivery of indicated services to hospice patients and families. He/she provides nursing services as identified and provided in accordance with the Plan of Care and with recognized standards of practice.
*Principal duties and accountabilities include, but are not limited to:
1. Participates as a member of the interdisciplinary team.
2. Develops a detailed Plan of Care for each assigned patient in conjunction with IDT, patient and family.
3. Assures all services indicated in the Plan of Care are provided, including coordinating services of other health care providers.
4. Reports to the attending physician and hospice medical director any significant changes in the patient’s condition and requests change of orders as needed.
5. Informs patients and family about the services included in the plan of care, including their proposed frequency.
6. Informs the patients and families in advance of any changes in the plan of care.
7. Implements medical treatment orders and patient follow up.
8. Teaches, supervises, and counsels patient/family regarding the nursing care needs, related concerns/issues of patient, services available and the Medicare Hospice Benefit.
9. Supervises Hospice Aides.
10. Completes all required documentation.
11. Participates in on-call duties and admissions as necessary.
12. Participates in in-service training for staff and contracted facilities.
13. Complies with the state and federal regulatory requirements.
*Qualifications:*
1. Registered Nurse with current state license.
2. Complies with acceptable professional standards and practice.
3. Demonstrates good communication.
4. Requires reliable transportation, current driver’s license and current auto insurance.
5. Must have Hospice experience
Job Type: Full-time
Pay: $80,000.00 - $85,000.00 per year
