Overview
Join our compassionate hospice care team as a dedicated Hospice Caregiver, where your role is vital in providing comfort, dignity, and personalized support to patients nearing the end of life. Your expertise in patient observation, medication administration, and assisting with activities of daily living (ADLs) will ensure our patients receive the highest quality of compassionate care. Duties
- Provide attentive patient observation to monitor health status and respond promptly to changes in condition
- Assist with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and ensuring patient comfort
- Administer medications accurately following medical documentation protocols while adhering to HIPAA regulations for patient privacy
- Support patients with dementia, Alzheimer's, memory care, and other cognitive impairments through specialized care techniques
- Maintain detailed medical records and document vital signs, catheter care, medication administration, and other medical procedures using EMR (Electronic Medical Records) or EHR systems
- Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe environment for patients in their homes or assisted living facilities
- Collaborate with interdisciplinary teams to ensure comprehensive hospice care plans are followed effectively
Experience
- Prior experience in hospice care or home health environments preferred but not required; training provided for motivated candidates
- Demonstrated ability to work with individuals with disabilities or developmental disabilities in various settings such as nursing homes or assisted living communities
- Knowledge of medical terminology relevant to hospice and senior care settings
- Hands-on experience with medical equipment including catheters, Hoyer lifts, vital signs monitoring, and CPR certification preferred
- Strong understanding of HIPAA compliance and confidentiality standards in healthcare settings
- Experience working with EMR/EHR systems for accurate documentation of patient care activities
- Long-term care experience or home & community care background highly valued

Pay: $55.00 - $70.00 per hour
