Hospice Clinical Liaison

Duties

  • Act as the primary point of contact for referral sources, hospitals, nursing homes, and community organizations to promote hospice services.
  • Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations to determine eligibility for hospice care.
  • Educate patients and families about hospice options, benefits, and the care process with clarity and compassion.
  • Collaborate with interdisciplinary teams to develop individualized care plans aligned with patient needs and goals.
  • Facilitate discharge planning by coordinating with hospital staff, case managers, and outpatient providers to ensure smooth transitions into hospice care.
  • Maintain detailed documentation of patient interactions, assessments, and referrals in medical records while adhering to HIPAA regulations.
  • Engage in utilization management by evaluating diagnostic evaluations and supporting appropriate case management strategies for optimal patient outcomes.

Requirements

  • Proven experience in critical care settings such as ICU, emergency medicine, or trauma centers; Level I or Level II trauma center experience highly preferred.
  • Strong background in hospital medicine, post-acute care, or outpatient clinics with familiarity in inpatient and outpatient settings.
  • Knowledge of medical terminology, physiology, diagnostic evaluation procedures, and medical records management.
  • Experience with discharge planning, intake processes, case management, and utilization management within acute or post-acute environments.
  • Excellent patient service skills combined with the ability to assess complex medical cases involving geriatrics, behavioral health, pediatrics (PICU), or infectious disease care.
  • Ability to navigate hospital experience including emergency room procedures and managed care protocols effectively.
  • Familiarity with HIPAA compliance standards and medical-surgical practices essential for maintaining confidentiality and delivering high-quality patient care.

Pay: $67,890.40 - $81,760.48 per year
