Duties

• Assist patients with activities of daily living such as bathing, grooming, dressing, and mobility support using equipment like Hoyer lifts when necessary
• Monitor and record vital signs, patient observations, and changes in condition accurately within electronic health record (EHR) systems such as Epic or Cerner
• Support medication administration under supervision, ensuring adherence to HIPAA regulations and safety protocols
• Provide emotional support and companionship to patients experiencing dementia or Alzheimer's care needs, including memory care strategies
• Collaborate with interdisciplinary teams to develop and implement individualized care plans for hospice patients
• Maintain infection control standards through aseptic technique and proper medical documentation
• Assist with medical procedures such as catheter care, phlebotomy, and basic life support during emergencies
