Job Overview

One Health Hospice, Inc. is seeking a motivated, relationship-driven, and compassionate Hospice Community Liaison/Marketer to help grow our presence throughout Alameda County, Contra Costa County, and San Joaquin County.

This position is ideal for someone with experience in hospice, home health, skilled nursing, healthcare marketing, or community outreach who enjoys building professional relationships and helping connect patients and families with compassionate end-of-life care services.

The Community Liaison will serve as the face of our hospice agency within the community by developing referral relationships with hospitals, skilled nursing facilities, assisted living facilities, physicians, case managers, discharge planners, and other healthcare professionals.

Responsibilities

• Develop and maintain referral relationships with healthcare providers and community partners
• Conduct in-person visits to hospitals, SNFs, ALFs, board & care facilities, physician offices, and community organizations
• Educate referral sources about hospice eligibility, services, and benefits
• Coordinate with clinical staff regarding patient referrals and admissions
• Participate in community events, networking opportunities, and outreach activities
• Maintain accurate records of marketing activities and referral trends
• Identify opportunities for census growth and strategic partnerships
• Represent the agency professionally and compassionately at all times

Qualifications

• Experience in hospice, home health, healthcare marketing, admissions, or case management preferred
• Strong communication and relationship-building skills
• Knowledge of hospice philosophy and Medicare hospice benefit preferred
• Self-motivated with ability to work independently
• Reliable transportation and willingness to travel within assigned territories
• Professional appearance and demeanor

Expected Salary Range

• $65,000–$110,000/year
• Additional performance-based bonuses may be available depending on referral growth and experience
