Shift is FRIDAY 5:00 PM - MONDAY 8:00 AM. This is a full-time position in the Kingfisher area. Travel: Up to 100%
RN Duties/Responsibilities:

Oversees patient cases from admission to death/discharge.
-
Completes all nursing assessments for each patient assigned.
-
Responsible for maintaining patient medication profiles, updating as needed, while ensuring updated profile is in patient charts at the office, as well as in the facility or home.
-
Updates the plan of care for each assigned patient every two weeks.
-
Completes the re-certification process by assessing and accurately documenting findings on appropriate forms.
-
Makes RN supervisory visits every 14 days on all assigned patients.
-
Completes admissions as assigned.
-
Provides on-call hours as determined by Patient Care Manager.
-
Accurately communicates patients’ information to Patient Care Manager and medical staff.
-
Participates in sharing of knowledge through in-services, staff meetings, and assisting in orientation of new personnel.

Performs routine duties without physical supervision.
-
Recognizes and responds to priorities in the patient care process.
-
Makes sound judgements based on factual information, while seeking guidance and direction as needed for performance of all duties.

Provides support to patients and families in a courteous manner with a positive tone.

Assesses, plans, implements, and evaluates patient care.

Provides nursing care to patients according to established policies and procedures.

Checks status of medications provided by Hospice at each visit, and reorders as needed.

Takes and records vital signs of the patient in an accurate and precise manner.

Reports and records any changes in condition or behavior of the patient.

Communicates concerns and/or changes to all disciplines.

Any and all other duties and responsibilities as assigned.
LPN Duties/Responsibilities:

Assists in the assessment, planning, implementation, and evaluation of patient care within the nursing care plan.

Provides nursing care to patients according to established policies and procedures.
-
Uses proper techniques, procedures, and safety precautions to meet individual needs of patients.

Checks status of medications provided by Hospice, and reorders as needed.
-
Performs skilled nursing visits as scheduled.
-
As authorized, prepares for set-up, hangs, monitors, and discontinues intravenous fluids with accuracy according to direction.

Provides direct care to patients, ensuring safety and comfort of the patient.
-
Performs treatments and/or assists in treatments and procedures with attention to detail, accuracy, patient safety, and comfort.
-
Takes and records vital signs of the patient in an accurate and precise manner.
-
Reports and records any changes in condition or behavior of the patient.
-
Records all care provided, following established guidelines for charting.

Utilizes the nursing process, and is familiar with other duties, recognizing their importance to patient care.

Participates in patient and family teaching and instruction.

Responsible for completion of assignments while maintaining quality work.

Able to adapt to changes in workload or patient assignments without interference to patient care.

Able to perform in an accurate and precise manner in emergency situations when time is of the essence.

Establishes good rapport with patients, families, and visitors, respecting patients rights to privacy and confidentiality of information.

Communicates pertinent information regarding patient to Interdisciplinary Team, using nursing care plan approach.

Provides support to patients and families in a courteous manner with a positive tone.

Independently performs assigned duties, including after-hours calls.

Participates in sharing of knowledge through in-service, staff meetings, and assisting in orientation of new personnel.

Makes sound judgments based on factual information, seeking guidance and direction, when necessary, for performance of duties.

Performs routine duties accurately without physical supervision.

Assist in maintaining equipment assigned to patient care, and reports malfunctioning of equipment to appropriate personnel.

RN Education, Licenses, Certifications and Experience:
-
Current Registered Nurse license.
-
Current CPR Certification.
-
1-2 years of experience in home health or hospice, LTCF preferred.
LPN Education, Licenses, Certifications and Experience:
-
High School Diploma, or GED equivalent.
-
Licensed Practical Nurse Certification.
-
Valid CPR Certification.
-
1 year of experience in home health, LTCF, or related field nursing preferred.
