We’re looking for a driven, ethical, and relationship-focused Hospice Liaison to grow our presence across the Dallas–Fort Worth area.

What You’ll Do:

• Build and maintain strong relationships with hospitals, SNFs, ALFs, and physician offices
• Educate families and referral partners on hospice services and patient choice
• Coordinate referrals and ensure smooth admissions
• Represent Gentle Transitions Hospice with integrity and professionalism in the community
• Participate in facility events, in-services, and outreach activities

Important Requirement (No Exceptions):
This is a full-time, dedicated role.
You may NOT work for or represent any other hospice, home health agency, or healthcare company while employed with GTH.

Qualifications:

• Hospice or healthcare marketing experience preferred
• Strong relationships in the Dallas–Fort Worth market
• Excellent communication and follow-up skills
• Self-motivated, organized, and results-driven
• Passion for serving patients and families at end of life
