Hospice Licensed Master Social Worker

Job Overview
We are seeking a compassionate and dedicated Hospice Licensed Master Social Worker or Licensed Bachelor Social Worker to join our interdisciplinary team. In this vital role, you will provide comprehensive social work services to patients and their families facing serious illness, ensuring emotional support, practical assistance, and guidance through complex healthcare decisions. Your expertise will help improve quality of life, facilitate effective care planning, and promote dignity in end-of-life experiences. Responsibilities

  • Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social support systems.
  • Develop individualized care plans that incorporate clinical counseling, behavioral therapy, and motivational interviewing techniques tailored to each patient’s circumstances.
  • Assist patient and family with DNR, MPOA, TODD, and obtaining provider service eligibility as needed.
  • Coordinate with healthcare teams to ensure seamless integration of hospice care services while maintaining compliance with HIPAA regulations and medical documentation standards.
  • Assist families with crisis intervention, case management, and navigating community resources such as assisted living or home & community care programs.
  • Maintain accurate medical records using EMR systems, documenting all patient interactions and treatment plans clearly and thoroughly.
  • Support research initiatives related to hospice care practices or behavioral health interventions to enhance service quality.

Qualifications

  • Master’s degree in Social Work (MSW) or Bachelors degree in Social work from an accredited program; licensure as a Licensed Master Social Worker (LMSW) or Bachelor Social Worker (LBSW) equivalent required.
  • Proven experience in hospice care or palliative settings preferred; inpatient or nursing home experience highly valued.
  • Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
  • Familiarity with behavioral health modalities such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and crisis management techniques.
  • Experience working with diverse populations including children, seniors, individuals with disabilities or developmental disabilities, Alzheimer’s disease, dementia, autism spectrum disorder, or PTSD.
  • Excellent communication skills with the ability to perform diagnostic evaluations and deliver compassionate mental health counseling.
  • Ability to work independently as well as part of an interdisciplinary team; proficient in EMR systems and data collection methods.
