Hospice Licensed Master Social Worker

Job Summary
We are seeking a compassionate and dedicated Hospice Licensed Master Social Worker to join our interdisciplinary care team. Responsibilities

  • Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social support systems.
  • Develop individualized care plans that incorporate social work interventions aligned with hospice care principles.
  • Provide clinical counseling and psychotherapy to patients and families dealing with grief, loss, chronic illness, or trauma, utilizing techniques such as motivational interviewing and cognitive behavioral therapy.
  • Facilitate individual and group therapy sessions focused on emotional well-being, adjustment to illness, and end-of-life planning.
  • Coordinate with healthcare teams to ensure seamless integration of social work services into overall patient care; document all interactions accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations.
  • Assist with crisis intervention as needed, providing immediate support during emergencies or acute emotional distress.
  • Support families through case management activities including resource linkage, advance directives, and post-death bereavement follow-up.
  • Maintain compliance with medical documentation standards and stay current with healthcare regulations such as ICD coding and CPT coding for billing purposes.

Experience

  • Master’s degree in Social Work (MSW) from an accredited institution required; licensure as a Licensed Master Social Worker (LMSW) or equivalent in the relevant state required.
  • Proven experience in hospice care or palliative settings preferred; experience working with diverse populations, including geriatrics, individuals with disabilities, or those facing terminal illnesses such as dementia or cancer strongly desired.
  • Familiarity with medical terminology, medical records management, and clinical research methodologies is highly advantageous.
  • Demonstrated ability to perform comprehensive patient assessments and deliver behavioral health interventions such as behavioral therapy, assisted living support, crisis management, and addiction counseling.
  • Knowledge of EMR systems and familiarity with ICD-10/ICD-9 coding standards for accurate documentation and billing processes.

Pay: $38.04 - $45.81 per hour
