Hospice Licensed Practical Nurse (LPN)

University Home Care Inc is seeking a dedicated Hospice Licensed Practical Nurse (LPN) for an in-office position serving Livonia and Royal Oak, MI. This role involves providing compassionate hospice care to patients, ensuring comfort and quality of life in their homes and in-office settings.

Responsibilities

• Provide direct nursing care to hospice patients according to care plans.
• Collaborate with interdisciplinary team members to coordinate patient care.
• Monitor and report changes in patient condition to the healthcare team.
• Educate patients and families on care techniques and symptom management.
• Complete and maintain accurate clinical documentation.
• Participate in patient and family meetings and discharge planning.
• Assist with in-office patient visits and support nursing staff in Livonia and Royal Oak locations.

Requirements

• Valid LPN license in the state of Michigan.
• Previous hospice or palliative care experience preferred.
• Strong communication and interpersonal skills.
• Ability to work both independently and as part of a multidisciplinary team.
• Reliable transportation for potential travel between Livonia and Royal Oak offices.
• Basic computer proficiency for documentation purposes.
