<div><div><div>We are seeking a full-time LVN Licensed Vocational Nurse to join our team! You will be responsible for delivering high-quality care to assigned Hospice patients. Clinical experience is preferred and candidates with Hospice experience are highly desirable.</div><br>

<b>Responsibilities:</b><br>
</div><ul><li>Provide Skilled Nursing Care in a home or facility setting</li>
<li>Ensure the appropriate use of meds, DME, and supplies</li>
<li>Educate and support patients, families, and caregivers</li>
<li>Participate in IDT and updating the Plan of Care</li>
<li>Provide our patients and families the best care possible</li>
<li>Daily travel is required when working</li>
</ul><div><b>Qualifications:</b><br>
</div><ul><li>Valid LVN License in California</li>
<li>One year or more of experience in either Home Health or Hospice is preferred, but not required</li>
<li>Previous experience in a hospice setting is a plus</li>
<li>Ability to build rapport with patients</li>
<li>Compassionate and caring demeanor</li>
<li>Excellent written and verbal communication skills</li></ul></div></div>
