Job Overview
We are seeking a compassionate and dedicated Hospice Licensed Master Social Worker (LMSW) to join our interdisciplinary care team. In this vital role, you will provide comprehensive psychosocial support to patients and their families facing serious illness, ensuring they receive holistic, patient-centered care. Responsibilities

  • Conduct thorough patient assessments, including psychosocial evaluations and mental health screenings, to inform personalized care plans.
  • Provide individual, family, and group counseling sessions utilizing evidence-based approaches such as cognitive behavioral therapy, motivational interviewing, and clinical counseling techniques.
  • Collaborate with medical teams to coordinate hospice care services, ensuring compliance with HIPAA regulations and accurate medical documentation.
  • Assist patients and families with crisis intervention, grief support, and advanced care planning discussions.
  • Manage case documentation within electronic medical records (EMR) systems, maintaining detailed records of patient interactions, diagnoses (ICD-10/ICD-9 coding), and treatment progress.
  • Facilitate intake procedures for new patients, including data collection on medical history, social factors, and current needs related to hospice care.
  • Engage in community outreach and education about hospice services, working with diverse populations such as seniors, individuals with disabilities or dementia, children in pediatric palliative settings, and those requiring post-acute or inpatient care.

Skills

  • Strong background in social work with experience in hospice care, behavioral health, mental health counseling, or related fields.
  • Proficiency in child & family counseling, crisis management, and behavioral therapy modalities like applied behavior analysis (ABA) or cognitive behavioral therapy (CBT).
  • Knowledge of medical terminology.
  • Experience working with individuals with developmental disabilities, autism spectrum disorder (ASD), Alzheimer’s disease or dementia care needs.
  • Familiarity with EMR systems for documentation and data collection; understanding of HIPAA compliance standards.
  • Ability to perform diagnostic evaluations and case management for diverse populations including geriatrics and pediatrics.
  • Excellent communication skills for intake assessments, psychotherapy sessions, group therapy facilitation, and interdisciplinary collaboration.
  • Capacity to handle crisis situations effectively through crisis intervention techniques and trauma-informed approaches.
