Duties

  • Perform comprehensive patient assessments including vital signs, physical examinations, and symptom evaluations to develop personalized care plans.
  • Administer medications, including injections and IV infusions, following strict protocols to manage pain and other symptoms effectively.
  • Provide wound care, catheterization, tube feeding, and ventilator management to support patient comfort and safety.
  • Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems such as Epic or Cerner to document patient care accurately and efficiently.
  • Educate patients and families on disease processes like Alzheimer's, dementia, or infectious diseases, empowering them with knowledge for better home care management.
  • Assist with discharge planning and coordinate services such as physical therapy or hospice resources to ensure seamless ongoing care.
  • Maintain compliance with HIPAA regulations, infection control standards, and state-specific nursing practices while delivering compassionate hospice care.

Experience

  • Valid LVN license with current state certification required; additional certifications in IV infusion, basic life support (BLS), and sterile processing are preferred.
  • Critical care experience such as ICU or emergency room settings is highly valued for managing complex symptoms in end-of-life scenarios.
  • Prior experience working in hospice, home health, long-term care facilities—including assisted living or nursing homes—is strongly preferred.
  • Demonstrated proficiency with EMR/EHR systems like Athenahealth or eClinicalWorks for accurate documentation of patient records.
  • Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation enhances your ability to deliver precise patient assessments.
  • Experience working with individuals with disabilities or developmental delays is a plus; familiarity with dementia care and memory support strategies is beneficial.
