Job Overview
Holy Care Hospice Inc. is seeking an experienced Hospice & Home Health Marketing Specialist to support our growth by developing strong referral relationships with hospitals, skilled nursing facilities, rehab centers, physicians, assisted living facilities, and community partners.
We are looking for someone who is well-connected, understands the healthcare referral landscape, and can professionally represent our organization in the community.
Responsibilities Primary Goal
Increase community awareness of Holy Care Hospice Inc. and drive overall census growth through professional outreach and relationship-building (not compensation tied to individual referrals).
Daily Duties
- Conduct daily outreach visits to hospitals, SNFs, LTACs, clinics, and ALFs.
- Meet with discharge planners, case managers, social workers, and physicians.
- Present Holy Care Hospice services clearly, professionally, and compliantly.
- Build, strengthen, and maintain long-term referral relationships.
- Participate in morning huddles or interdisciplinary meetings when requested.
- Follow up on pending referrals and provide timely communication to office staff.
- Coordinate with the office regarding admission packets, eligibility checks, and updates.
- Maintain accurate weekly activity logs and referral tracking reports.
Partnership & Contracting Support
- Assist in developing networking agreements with hospitals and skilled nursing facilities.
- Support connections with Medi-Cal plans, IPA groups, and managed care entities.
- Identify new markets, community partners, and referral opportunities.
- Support the leadership team with outreach strategy and growth initiatives.
Requirements
- Required: Experience in hospice or home health marketing / community liaison work.
- Existing relationships with hospital case managers or SNFs is highly preferred.
- Strong communication, networking, and relationship-building skills.
- Understanding of Medicare/Medi-Cal processes and general referral workflows.
- Ability to travel daily for outreach.
- Valid driver’s license and reliable vehicle.

Pay: $55,000.00 - $160,000.00 per year
Work Location: On the road
