Hospice Nurse Practitioner

Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and symptom evaluations to develop personalized care plans.
  • Manage complex symptom control, including pain management, IV infusion therapy, and acute care interventions tailored to hospice patients.
  • Collaborate with interdisciplinary teams to coordinate hospice services, discharge planning, and case management strategies that align with patient needs and preferences.
  • Provide education and emotional support to patients and their families about disease progression, medication administration (including CPT coding for billing), and advanced directives.
  • Document all clinical encounters accurately within electronic health record (EHR) systems such as Epic or eClinicalWorks, ensuring compliance with HIPAA regulations.
  • Administer injections, perform suturing when necessary, and utilize sterile processing techniques to maintain high standards of patient safety.
  • Participate in telehealth consultations, triage calls, and remote patient monitoring to extend care beyond traditional settings.

Qualifications

  • Current Nurse Practitioner license with certification in hospice & palliative medicine or related specialty.
  • Proven experience in critical care settings such as ICU or emergency medicine is highly valued.
  • Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth; telehealth experience is a plus.
  • Strong knowledge of medical terminology, physiology, ICD coding (ICD-10/ICD-9), CPT coding, and medication administration protocols.
  • Experience working with diverse populations including geriatrics, pediatrics (including neonatal ventilator management), individuals with disabilities or developmental disabilities.
  • Skills in pain management techniques such as dermal fillers or botulinum toxin treatment are advantageous but not required.
  • Ability to perform physical examinations, venipuncture, catheterization, sterile procedures, and basic life support (BLS).
  • Excellent communication skills for patient education, family support, and interdisciplinary collaboration. Pay: $50.00 - $55.00 per hour
