Hospice Nurse Practitioner

FG Healthcare Services is seeking an experienced Hospice Nurse Practitioner to provide

advanced clinical care and support to our hospice patients and families. This is an excellent

opportunity for an NP with a passion for palliative care to work within a compassionate

interdisciplinary team while maintaining flexible, per-diem scheduling.

What You'll Do

• Conduct comprehensive patient assessments and face-to-face eligibility visits as required by Medicare
• Collaborate with the hospice physician and IDT to develop and update plans of care
• Manage complex symptoms including pain, dyspnea, nausea, and anxiety
• Order and adjust medications and treatments within scope of practice
• Provide education and emotional support to patients and families on disease progression and comfort goals
• Participate in IDT meetings and contribute clinical expertise to care planning
• Complete accurate, timely documentation in compliance with hospice regulations

Requirements

• Active Michigan NP license / APRN certification (required)
• MSN or DNP from an accredited program
• Hospice, palliative care, or geriatric/oncology NP experience strongly preferred
• ACHPN certification a plus
• Familiarity with Medicare hospice Conditions of Participation
• Reliable transportation and valid Michigan driver's license

Pay: $91.00 per hour
