Hospice Nurse Practitioner (NP) – In-Home Hospice & Palliative Care

Job Overview

Hospice & Beyond Palliative Care LLC is seeking a compassionate, experienced, and highly independent Hospice Nurse Practitioner (NP) to provide in-home hospice and palliative care services to patients throughout Houston and surrounding areas. The ideal candidate must be comfortable working autonomously in the field, conducting face-to-face patient visits, managing symptoms, prescribing medications, and collaborating closely with the interdisciplinary hospice team to ensure quality end-of-life care.

This role requires a provider who is clinically confident, organized, compassionate, and able to travel throughout the Houston metropolitan area for patient visits.

Must have an active collaborating/supervising physician arrangement as required by Texas law

Pay: $75 -$120/ Visit

Responsibilities

  • Perform comprehensive hospice and palliative care assessments in patients’ homes
  • Conduct face-to-face (F2F) encounters for hospice recertifications in compliance with Medicare/CMS guidelines
  • Evaluate and manage pain and other distressing symptoms related to terminal illness
  • Prescribe medications, treatments, and interventions appropriate for hospice patients
  • Develop and update individualized plans of care in collaboration with the hospice interdisciplinary team (IDG)
  • Collaborate with the Medical Director, nursing staff, social workers, chaplains, and other healthcare professionals
  • Provide education and emotional support to patients and families regarding disease progression, symptom management, medications, and hospice philosophy
  • Participate in interdisciplinary group (IDG) meetings and clinical discussions
  • Review and document clinical findings accurately and timely within the EMR system
  • Assist with advance care planning and goals-of-care discussions
  • Respond to urgent clinical concerns as needed
  • Ensure compliance with Medicare, state, federal, and hospice regulatory requirements
  • Support hospice admissions and transitions of care when appropriate
  • Maintain professional communication with hospitals, nursing facilities, assisted living communities, and referral partners
  • Travel throughout Houston and surrounding communities for scheduled patient visits

Qualifications

  • Current and unrestricted Texas Nurse Practitioner license
  • Board-certified Nurse Practitioner (FNP, AGNP, AGACNP, or related specialty)
  • DEA license with prescriptive authority required
  • Must have an active collaborating/supervising physician arrangement as required by Texas law
  • Ability to prescribe medications independently within scope of practice
  • Minimum of 2 years of hospice, palliative care, home health, geriatrics, oncology, or critical care experience preferred
  • Strong assessment and symptom management skills
  • Knowledge of hospice Medicare Conditions of Participation (CoPs) preferred
  • Experience with EMR documentation systems preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently with minimal supervision
  • Reliable transportation and valid Texas driver’s license required
  • Willingness to travel throughout Houston and surrounding areas

  • Proven experience in hospice & palliative medicine with a strong foundation in critical care or acute pain management.
  • Proficiency in using EMR/EHR systems like Epic, Cerner, or Athenahealth for documentation and patient tracking.
  • Expertise in managing complex cases involving infectious disease care, dementia care, or geriatrics.
  • Skilled in performing clinical procedures such as suturing, catheterization, phlebotomy, sterile processing techniques, and sterile dressing changes.
  • Knowledge of medical terminology, anatomy knowledge, physiology principles, and diagnostic evaluation methods including laboratory experience.
  • Ability to work effectively with diverse populations including children (pediatrics), seniors (geriatrics), individuals with disabilities or developmental disabilities.

Strong communication skills for family education, interdisciplinary collaboration, and team leadership in high-pressure environments such as hospital medicine or Level I Trauma Centers. Pay: $75.00 - $120.00 per hour
