Overview
Join our compassionate healthcare team as a Hospice Nurse Practitioner PRN, where you will play a vital role in delivering high-quality, patient-centered end-of-life care. This flexible position offers the opportunity to work with diverse patient populations, including those in assisted living, nursing homes, and home care settings. As a key member of our interdisciplinary team, you will provide expert clinical assessments, symptom management, and emotional support to patients and their families during challenging times. Your expertise in hospice and palliative medicine will help ensure comfort, dignity, and respect for every individual under your care.
Responsibilities
- Conduct comprehensive patient assessments to evaluate physical, emotional, and spiritual needs within hospice settings.
- Face to Face visits for newly admitted patients and required recertifications. Make sure they are done timely prior to the Recertifications.
- Collaborate with physicians, social workers, chaplains, and other team members to coordinate holistic care.
- Provide education and support to patients’ families on disease progression, medication use, and comfort measures.
- Document all patient interactions accurately using EMR systems HCHB while adhering to HIPAA regulations.
Requirements
- Current Nurse Practitioner license with certification in hospice & palliative medicine or relevant specialty.
- Proven experience in hospice care or related fields such as geriatrics, senior care, or home health nursing.
- Demonstrated proficiency with EMR/EHR systems like Epic or Athenahealth; telehealth experience is a plus.
- Strong clinical skills including vital signs assessment, physical examinations, medication administration (including injections), and catheter care.
- Knowledge of medical terminology, CPT coding, ICD-10 coding, and utilization review processes.
- Critical care experience such as ICU or emergency room background highly desirable.
- Excellent communication skills with the ability to provide compassionate patient education and family support.

Pay: $45.05 - $54.25 per hour
