Overview
We are seeking a dedicated and experienced Hospice On-Call Registered Nurse (RN) to provide critical after-hours nursing support for our hospice patients. This role offers an opportunity to deliver compassionate end-of-life care, ensuring comfort, dignity, and quality of life for patients during evenings, nights, weekends, and holidays. The Hospice On-Call RN will work 7 On/7 Off, working 7 consecutive nights and covering weekends.
Responsibilities
- Answering incoming calls from patients, families, and caregivers
- Making emergency visits to patient homes or facilities to manage symptoms (pain, anxiety, breathing issues).
- Provide education to patients and family members regarding symptom management, medication administration, and advance directives.
- Updating electronic medical records with details of calls, visits, and interventions in real-time.
- Conducting urgent after-hours admissions when needed, ensuring a seamless transfer of care.
- Collaborate with interdisciplinary teams to develop or adjust individualized care plans aligned with patient wishes and clinical needs.
- Maintain up-to-date knowledge of hospice protocols, infectious disease control measures, and relevant healthcare regulations.
Qualifications
- Current RN licensure in the state of practice with valid CPR certification.
- Hospice experience is a MUST
- Strong assessment skills, crisis intervention abilities, and the ability to work independently, often at night or on weekends.
- Familiarity with medical records systems required.
- Excellent communication skills with the ability to provide clear patient education and collaborate effectively with healthcare teams.
- Ability to handle high-pressure situations calmly while demonstrating compassion and professionalism.
- Valid Drivers License
Pay: From $85,200.00 per year
