Hospice Patient Care Coordinator- LVN Required

We are seeking a mission-driven Patient Care Coordinator (LVN) who thrives in a fast-paced, growth-oriented hospice environment. This role is central to our internal clinical operations and works in close partnership with Clinical Leadership to ensure excellence in patient care delivery, regulatory compliance, and seamless interdisciplinary collaboration.

As our Patient Care Coordinator (LVN), you will:

  • Strengthen internal processes that directly impact patient outcomes.
  • Support timely assessments, recertifications, and documentation compliance.
  • Ensure communication flows efficiently between clinical teams, physicians, families, and office staff.

Core Responsibilities

Clinical Operations & Leadership Support

  • Partner closely with the Director of Patient Care Services and Clinical Leadership to manage daily patient care operations.
  • Review clinical documentation for timeliness, accuracy, and regulatory compliance.
  • Support coordination of admissions, recertifications, IDT collaboration, and discharge planning.
  • Monitor patient status trends and escalate concerns proactively.

Care Coordination

  • Collaborate with the interdisciplinary team (RN, MSW, CHHA, Chaplain, MD) to ensure individualized care plans are executed effectively.
  • Serve as a communication hub between field clinicians and office operations.
  • Coordinate physician communication, orders, and follow-up.

Quality & Compliance

  • Ensure documentation meets Medicare and accreditation standards.
  • Track key quality indicators (live discharges, hospitalizations, timely visits).
  • Support audit readiness and internal process improvements.

Patient & Family Support

  • Provide education regarding hospice services, symptom management, and plan of care.
  • Assist with addressing concerns or service gaps promptly and compassionately.
  • Uphold patient dignity, autonomy, and comfort at all times.

Administrative & Systems Management

  • Utilize EHR systems to track, document, and communicate care updates.
  • Assist with scheduling coordination as needed.
  • Maintain organized and compliant patient records.

Qualifications

  • Active California LVN license (required).
  • Minimum 2 years of clinical experience; hospice or palliative care strongly preferred.
  • Strong understanding of hospice regulations and Medicare guidelines.
  • Experience with EHR systems and clinical documentation oversight.
  • Exceptional organizational skills and ability to manage multiple priorities.
  • Clear, confident communication style with clinical and non-clinical teams.
  • Ability to function in a high-growth, evolving environment with professionalism and resilience.

Pay: $63,000.00 - $80,000.00 per year
