We are a well-established hospice company looking to add to our growing team. Preferred Hospice has 5 offices in Missouri serving over 65 counties total in the state. Those offices are in St Louis, Farmington, Dexter, Columbia, and Ozark.
We are currently looking for an experienced Hospice Physician to provide oversight to the Columbia, MO office. This MD or DO would provide leadership over the Interdisciplinary Group in Columbia.
The Columbia office is a busy office that provides high quality hospice care to patients within a 60+ mile radius of Columbia.
Regulated meetings are every 14 days.
The Preferred Hospice of Missouri L.L.C. Hospice Physician must be a doctor of medicine or osteopathy who is trained in pain control and symptom management and is familiar with the psychosocial aspects of hospice care.
The Hospice Physician:
Qualifications:
· Current Physician license and is in good standing with the State of Missouri.
· Current National Provider Identification Number
· Current DEA form 224a
· Professional or personal experience in death and grief, with an ability to recognize the positive and negative aspects.
· Exhibits knowledge of palliative care and a holistic approach to patient care.
· Ability to assess and evaluate patients for hospice eligibility based on the approved criteria and guidelines set forth by CMS.
· Understanding the criteria of certifying and re-certifying patients based on assessment.
JOB RESPONSIBILITIES:
· Face to Face visits/assessments (where applicable) of the Hospice patients and other duties outlined in the collaborative agreements.
· Maintaining liaison between the Hospice, the skilled facilities, and the community.
· Providing quality educational services to all employees, facilities, and patients/families of the Hospice Agency.
· Participating in development and implementation of the Hospice quality performance improvement program as required by state and federal regulation.
· Providing education to skilled facilities on hospice eligibility criteria and how to identify potential patients to receive the hospice benefit.
· Oversight, guidance and participation in IDG meetings.
· Collaboration and consultation to other hospice physicians in the organizational NPI.
· Implementation or agreement of physician’s standing orders to be reviewed yearly and approved with signature.
· Available for physician consultation for patients 24 hours per day when clinician seeks medication orders outside the physician approved standing orders.
· The Hospice Physician is responsible for attending and consult for each team meeting assigned.
· Ability and willingness to perform job duties within current Electronic Medical Record (EMR) software and iPrescribe/Backline HIPAA compliant messaging.
