Audubon Hospice of Lafayette is expanding and searching for a RN case Manager to care for our patients in the Greater Acadiana Area. In this vital role, you will deliver high-quality, personalized nursing care in a home or community setting, ensuring patients' physical, emotional, and spiritual needs are met with empathy and professionalism. Your proactive approach will help ease pain, manage symptoms, and promote quality of life through comprehensive care planning and coordination.
Responsibilities
- Conduct thorough patient assessments to evaluate physical condition, pain levels, and emotional well-being using advanced clinical judgment.
- Develop individualized care plans in collaboration with interdisciplinary teams, incorporating hospice medicine best practices.
- Provide education and emotional support to patients and families about disease progression, symptom management, and end-of-life options.
- Monitor patient progress regularly through vital signs, physical examinations, and diagnostic evaluations; adjust care plans accordingly.
- Maintain accurate medical documentation within electronic health record (EHR) systems such as Wellsky while ensuring compliance with HIPAA regulations.
- Participate in team meetings, case management activities, and ongoing training to stay current with industry standards
Experience
- Proven experience in hospice & palliative medicine or related fields such as critical care, ICU experience, or emergency medicine.
- Demonstrated proficiency with EMR/EHR systems for accurate documentation and patient tracking.
- Strong background in managing complex patient needs involving acute pain management, infection control, and symptom relief.
- Familiarity with medical procedures such as catheter care, ostomy care, ventilator management, and tube feeding is highly desirable.
- Experience working with diverse populations including individuals with developmental disabilities, dementia care (Alzheimer’s), geriatrics, pediatric patients (including neonatal ventilator), or those in long-term care settings like nursing homes or assisted living facilities.
- Knowledge of medical terminology, anatomy knowledge, pharmacology principles for medication administration including physiology knowledge for comprehensive patient assessment.
- Certification in basic life support (BLS) and relevant licensure as a registered nurse (RN) is required; additional certifications such as IV insertion or advanced wound care are advantageous.
