Hospice Registered Nurse (RN) Case Manager

Pay Rate: $76,693 per year with room for advancement based on years of service with organization.

Position Summary

Registered Nurse (RN) Case Manager will coordinate patient care for a caseload of patients who are terminally ill throughout the CareFirstNY service area: Steuben, Chemung, and Schuyler counties. The Registered Nurse (RN) Case Manager will act as the primary staff member responsible for establishing and implementing the plan of care for each patient on their caseload, ensuring that communications educate, advocate, and empower patients/families, while performing nursing in fiscally responsible manner.

Essential Duties and Responsibilities

  • Share in on-call rotation, providing 24-hour, 7 day a week coverage to patients and families.
  • Provides information visit and admissions as assigned.
  • Complete assessment of patient/family needs, development of care plans, communicate/educate in a manner to advocate and empower, and complete nursing in a fiscally responsible manner.
  • Provide anticipatory guidance to patient/family as needed throughout the duration of hospice care.
  • Make follow up contact with family/caregiver after patient death to deal with any closure issues.
  • Participate in interdisciplinary group meetings to update patient/family plan of care as needed.
  • Participate in quality assurance program and perform chart audits as assigned.
  • Consult with Team Manager(s) or designee and/or other interdisciplinary team members before acting on changes in the patients’ health status, community referral potential, family stresses, and unanticipated nursing or medical needs.
  • Work with and collaborate with all members of the interdisciplinary team as indicated.
  • Maintain timely documentation according to agency guidelines.
  • Provide supervision and evaluation of hospice and contracted homemakers, home health aides, and LPNs.
  • Participate in staff development.
  • Other duties as assigned by supervisor.

Qualifications

Must possess a valid and current NYS Professional Registered Nurse (RN) license and driver’s license. Candidates should possess physical assessment skills, strong communication skills, and be skilled in use of nursing processes.

Additional experience(s)/education:

  • Experience in home care or acute care settings
  • Know and understand state and federal regulations relating to hospice and palliative care.
  • Ability to utilize critical thinking skills and work autonomously.
