Job Summary
We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our dedicated team. The Hospice RN provides skilled nursing care, emotional support, and guidance to patients and families during the end-of-life journey. This role focuses on comfort care, symptom management, and holistic support—delivered primarily in patients’ homes, assisted living communities, or nursing facilities. The ideal candidate demonstrates clinical expertise, empathy, and strong communication skills while working collaboratively with the hospice interdisciplinary team.

Duties

  • Conduct comprehensive nursing assessments of hospice patients, evaluating physical, emotional, and psychosocial needs.
  • Develop and implement individualized care plans focused on pain control, symptom management, and quality of life.
  • Administer medications and treatments as prescribed, ensuring comfort and adherence to hospice protocols.
  • Provide education and emotional support to patients and their families regarding disease progression, medications, and end-of-life care.
  • Collaborate closely with physicians, social workers, chaplains, certified nursing assistants (CNAs), and volunteers to ensure coordinated care.
  • Monitor changes in patient condition and communicate updates promptly to the interdisciplinary team.
  • Maintain accurate, timely documentation within the electronic health record (EHR).
  • Participate in on-call rotation as needed to ensure continuity of care.
  • Support the hospice philosophy by promoting dignity, respect, and compassion for patients and families.

Experience and Qualifications

  • Current, unencumbered Registered Nurse (RN) license in the state of practice.
  • Minimum of one year of nursing experience, preferably in hospice, home health, or palliative care.
  • Strong assessment and critical-thinking skills in a home or community-based setting.
  • Excellent communication and interpersonal skills with the ability to support patients and families through emotional and spiritual challenges.
  • Proficiency in electronic documentation (experience with EMR/EHR systems preferred).
  • Valid driver’s license, reliable transportation, and proof of insurance (as home visits are required).
  • Commitment to compassionate, patient-centered end-of-life care.

Job Type: Full-time

Pay: $75,374.36 - $90,773.42 per year
