Hospice RN Case Manager

*Job Overview*

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. This position requires strong clinical skills, excellent communication abilities, and a commitment to providing high-quality patient care.

*Responsibilities*

- Conduct comprehensive patient assessments to determine individual needs and develop tailored care plans.

- Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals to coordinate care effectively.

- Manage a caseload of patients, ensuring continuity of care across various settings such as managed care, behavioral health, and medical-surgical units.

- Monitor patient progress and outcomes, adjusting care plans as necessary to meet evolving health needs.

- Provide education and support to patients and their families regarding treatment options, resources, and health management strategies.

- Advocate for patients within the healthcare system to ensure they receive appropriate services and support.

- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.

*Experience*

- Current Registered Nurse (RN) license in good standing.

- Experience in managed care or case management.

- Background in behavioral health or geriatrics is a plus.

- Familiarity with patient assessment techniques and tools.

- Knowledge of physiology and medical-surgical nursing practices is essential.

- Experience working in high-acuity environments such as PICU or Level I Trauma Centers is advantageous.

- Previous experience in emergency room (ER) settings is beneficial but not required.

Job Type: Full-time

Pay: $80,000.00 - $90,000.00 per year
