Job Summary
The Hospice RN Case Manager/Admission Nurse is responsible for coordinating care for assigned patients, managing clinical needs, and serving as the primary nurse contact for patients and families. This role also provides leadership in the admission process by completing initial assessments, explaining hospice philosophy, and initiating the plan of care.

Essential Responsibilities

Admissions

· Respond promptly to referrals and schedule admission visits.

· Conduct comprehensive initial nursing assessments, including physical, psychosocial, and spiritual dimensions.

· Explain hospice services, philosophy, and eligibility criteria to patients and families. Ensure all medical records are present in chart to support admission diagnosis.

· Obtain consents, election of benefit, and other regulatory documents.

· Develop an initial plan of care in collaboration with the hospice physician and interdisciplinary team (IDT).

· Coordinate medication reconciliation and ensure delivery of medications, supplies, and durable medical equipment (DME).

· Provide immediate teaching to patients and families on hospice philosophy, symptom management, and available support.

Case Management

· Serve as the primary nurse responsible for assigned patients after admission.

· Complete ongoing nursing assessments, updating the plan of care to reflect current needs.

· Provide skilled interventions, including medication administration, wound care, and symptom management.

· Educate patients and caregivers on disease progression, comfort measures, and equipment use.

· Communicate changes in condition promptly to the physician and IDT.

· Coordinate care with hospice aides, social workers, chaplains, and therapists to ensure holistic support.

· Advocate for patient goals, dignity, and comfort.

Team Collaboration

· Lead IDT discussions for assigned patients, presenting updates on status and goals.

· Support hospice aides through supervisory visits and ongoing communication.

· Participate in orientation and mentoring of new staff.

Documentation & Compliance

· Document all assessments, interventions, communications, and education in the EMR accurately and timely.

· Ensure compliance with state, federal, and accreditation standards, including Medicare Conditions of Participation.

· Participate in Quality Assessment and Performance Improvement (QAPI) activities.

Professional Responsibilities

· Participate in the on-call rotation, responding to urgent patient and family needs.

· Maintain active RN license and required continuing education.

· Represent the organization professionally to patients, families, referral sources, and the community.

Qualifications

· Current, unrestricted RN license in the state of practice.

· Minimum of one year clinical nursing experience (hospice, palliative care, or home health preferred).

· Strong communication, teaching, and organizational skills.

· Ability to work independently and as part of an interdisciplinary team.

· Valid driver’s license, auto insurance, and reliable transportation.

Job Type: Temporary

Base Pay: $50.00 - $250.00 per hour

Expected hours: 1 – 20 per week
