Position Summary
The Hospice RN Case Manager is responsible for coordinating and providing skilled nursing care to hospice patients in their place of residence. This role includes comprehensive patient assessment, care planning, symptom management, patient/family education, and collaboration with the interdisciplinary team (IDT) to ensure comfort-focused, patient-centered care.
The ideal candidate is clinically strong, highly organized, emotionally intelligent, and confident managing patient needs across changing conditions.
Key Responsibilities
Patient Care & Clinical Management
- Completing of RN visit notes within 24-48hrs of visit
- Complete comprehensive nursing assessments, including physical, psychosocial, and symptom-related evaluations.
- Develop, implement, and update individualized Plan of Care in collaboration with the hospice physician and interdisciplinary team.
- Provide skilled nursing visits and interventions, including:
- Pain and symptom management
- Medication reconciliation and education
- Wound care (as applicable)
- Catheter care, ostomy care, and other skilled nursing needs
- Identify changes in patient condition and respond appropriately, including escalation to the hospice physician and/or IDT as needed.
- Provide ongoing patient and caregiver education on disease progression, comfort measures, medications, and what to expect.
Care Coordination & Case Management
- Coordinate services and communication among the interdisciplinary team, including:
- Hospice physician/medical director
- Social workers
- Chaplain/spiritual care
- Home health aides
- Volunteers
- Therapists (as appropriate)
- Collaborate with caregivers and family members to ensure continuity of care and support.
- Serve as the primary point of contact for assigned patients and families, ensuring timely follow-up and responsiveness.
Documentation & Compliance
- Maintain accurate and timely clinical documentation (24-48hrs interval) in the electronic medical record (EMR), including:
- Visit notes
- Patient assessments
- Care plan updates
- Medication and symptom tracking
- Ensure compliance with hospice regulations, agency policies, and standards of practice.
- Participate in IDT meetings and contribute to patient care discussions and goals.
Support for Patients and Families
- Provide emotional support and compassionate communication to patients and families during end-of-life care.
- Educate families on hospice philosophy, comfort care goals, and available support resources.
- Assist families with end-of-life planning, transitions, and bereavement resources in coordination with the hospice team.
On-Call (as assigned)
- Provide triage, guidance, and nursing visits as needed to manage symptoms and support families.
Qualifications
Required
- Current, active Registered Nurse (RN) license in Texas
- Minimum 5 years of RN experience (hospice, geriatrics, or palliative care preferred)
- Valid driver’s license and reliable transportation
- Current CPR certification (or ability to obtain upon hire)
- Ability to work independently while collaborating with a team
- Strong documentation and time-management skills
Preferred
- Hospice experience strongly preferred
- Experience with EMR documentation systems. (Kinsser Wellsky)
- Strong knowledge of end-of-life symptom management and comfort care
