We are seeking a strong, decisive, clinically sharp Hospice RN Case Manager who understands that leading care is not about checking boxes — it’s about owning outcomes.

What You’ll Own

You won’t just manage visits. You will lead the clinical direction of your caseload.

  • Perform comprehensive assessments that go beyond vitals — identifying risk, decline patterns, and opportunities for early intervention.
  • Develop and drive individualized care plans aligned with patient goals and hospice regulations.
  • Lead interdisciplinary collaboration with physicians, social workers, chaplains, aides, and leadership to ensure seamless, high-quality care.
  • Prevent unnecessary hospitalizations through proactive symptom management and family education.
  • Navigate complex discharge and transition planning across inpatient, outpatient, and post-acute settings.
  • Conduct detailed medical record reviews and ensure compliance with hospice Conditions of Participation.
  • Oversee admissions and ensure documentation excellence within EMR systems.
  • Educate families on symptom control, medication management, and advance care planning with clarity and confidence.
  • Partner with managed care organizations and facilities to ensure coordinated, value-based service delivery.

Who Thrives Here

  • RN with strong clinical foundation in hospital medicine, ICU, ED, trauma, post-acute, or complex case management.
  • Skilled in discharge planning, utilization management, and interdisciplinary leadership.
  • Strong understanding of geriatrics, infectious disease, behavioral health considerations, and pediatric hospice.
  • Proficient in medical terminology, documentation, regulatory compliance, and EMR systems.
  • Calm under pressure. Clear in communication. Accountable in action.

Pay: $115,000.00 - $130,000.00 per year
