The Hospice RN Case Manager is responsible for coordinating and managing the care of assigned hospice patients in accordance with the hospice philosophy, physician orders, interdisciplinary plan of care, and Medicare Conditions of Participation. The RN Case Manager ensures comprehensive, individualized care focused on comfort, symptom management, and quality of life for patients with a terminal diagnosis.
Clinical Responsibilities
- Perform initial and ongoing comprehensive assessments within required timeframes.
- Develop, implement, and update the individualized Plan of Care (POC).
- Coordinate care with the interdisciplinary team (IDG).
- Provide skilled nursing care including:
- Pain and symptom management
- Medication administration and education
- Wound care
- IV therapy (if applicable)
- Foley catheter care
- Assess and manage terminal symptoms (dyspnea, pain, agitation, nausea, etc.).
- Monitor patient decline and document disease progression clearly to support hospice eligibility.
- Ensure 24-hour on-call coverage per agency protocol.
- Educate patients and caregivers regarding disease process and end-of-life care.
- Facilitate DME, medication, and supply coordination.
Care Coordination & IDG
- Present assigned patients at bi-weekly IDG meetings.
- Collaborate with:
- Medical Director
- Social Worker
- Chaplain
- Hospice Aide
- Volunteer Coordinator
- Ensure timely updates of physician orders.
- Communicate changes in patient condition promptly.
Documentation Responsibilities
- Complete all documentation accurately and timely in EMR.
- Maintain compliance with Medicare and state documentation standards.
- Ensure documentation supports terminal prognosis (6-month life expectancy).
- Document visits, assessments, interventions, and patient response.
- Maintain medication reconciliation and controlled substance logs.
Compliance & Quality
- Adhere to agency policies and procedures.
- Participate in QAPI activities.
- Report incidents, complaints, and safety concerns.
- Maintain patient confidentiality (HIPAA compliant).
- Participate in survey readiness efforts.
Physical Requirements
- Ability to lift up to 50 lbs.
- Ability to stand, bend, and assist patients with mobility.
- Travel within service area required.
- Exposure to home environments and infectious conditions.

Pay: $82,000.00 - $95,000.00 per year
