Hospice RN-PRN

GENERAL PURPOSE

The PRN Registered Nurse (RN) provides hospice nursing care for Sincero Hospice patients. This role focuses on timely response to patient needs, effective symptom management, and clear communication with patients, families, and the interdisciplinary team in a fast-moving startup hospice environment.

QUALIFICATIONS

Current, active RN license in the state of employment

Minimum of one (1) year of hospice or end-of-life care experience preferred

Strong acute care and symptom-management skills

Demonstrates sound clinical judgment and ability to work independently

Clear, compassionate communication skills

Strong documentation and time-management abilities

Current CPR certification

Valid driver’s license and reliable, insured vehicle meeting state and Sincero Hospice requirements

Ability to perform essential job functions with or without reasonable accommodations

ESSENTIAL JOB FUNCTIONS

Patient Care

Complete urgent, PRN, and crisis visits as required

Provide skilled nursing interventions focused on pain and symptom management, comfort, and safety

Assess changes in patient condition and implement appropriate interventions

Administer medications and treatments as ordered by the physician

Provide emotional support and education to patients and families

Manage end-of-life care needs, including death visits when assigned

Communication & Coordination

Communicate patient status and significant changes to physicians and hospice leadership

Obtain and implement physician orders as needed

Coordinate care with hospice aides, social workers, chaplains, and other team members

Ensure smooth handoff and follow-up communication with the daytime clinical team

Documentation

Complete timely, accurate documentation for all contacts and visits

Document assessments, interventions, outcomes, and physician communication in compliance with hospice regulations

Additional Duties

Participate in scheduled on-call rotations, including nights, weekends, and holidays as assigned

Adhere to all Sincero Hospice policies, procedures, and regulatory standards

Perform other duties as assigned to support hospice operations

This role is essential to ensuring continuous, compassionate hospice care outside of regular business hours. Responsibilities may evolve as Sincero Hospice grows.

COMPENSATION & SCHEDULE

PRN on-call compensation structure (flat rate, per-visit, or hourly based on coverage needs)

Flexible scheduling aligned with agency coverage requirements

Pay: $30.00 - $50.00 per hour

Work Location: Hybrid remote in San Antonio, TX 78249
