HOSPICE RN SUPERVISOR ON-CALL- PART-TIME

The Martin and Edith Stein Hospice, an agency of The Oscar and Ella Wilf Campus for Senior Living, is currently in search of Hospice RN Supervisor On-Call to join our team. We are an out-patient hospice dedicated to quality assurance with an emphasis on a strong foundation of family, community, long-term commitment, and entrepreneurial spirit. We are located in Somerset, however, our patients are throughout Middlesex and surrounding counties.

HOSPICE RN SUPERVISOR- ON-CALL - PART-TIME

7 DAYS/WEEK EVERY OTHER WEEK

  • FRIDAY 5PM - SATURDAY 8AM (15 HRS)
  • SATURDAY 8AM - SATURDAY 8PM (12 HR)
  • SATURDAY 8PM - SUNDAY 8AM (12 HRS)
  • ON-CALL RATE $10.00/HR
  • ADDITIONAL PAY PER VISIT RATE
  • ADMISSION VISIT $200.00
  • PRONOUNCEMENT VISIT $100.00
  • RE-VISIT $75.00

The Hospice RN Supervisor On-Call ensures the process by which patients have access to hospice services 24 hours per day. On-Call Hospice services are provided after hours, including evenings, weekends, and holidays, by supervisory and clinical staff. The Hospice RN Supervisor On-Call provides supervisory support, coverage, and availability to the clinical staff to address the needs of hospice patients.

Duties and Responsibilities

  • Maintain availability by telephone during on-call hours.
  • Maintain telephone equipment in good working order.
  • Maintain availability to clinical staff, patients, and the answering service at all times.
  • Maintain On-Call Log of all communications and activity.
  • Respond to a page/call within 15 minutes and must be able to reach a patient within one (1) hour.
  • Communicate/Report on-call activity to the Clinical Director per standard operating procedure.
  • Provide the following services as needed:
  • Follow-up phone calls to staff, patients, family/caregivers, vendors.
  • Visit the patient, as necessary.
  • Obtain physician (or other authorized independent practitioner) orders, as needed.
  • Arrange for other hospice services, as needed.
  • Document each patient/family interaction in a clinical note.
  • Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physician’s (or other authorized independent practitioner’s) orders as required.
  • Assist the patient and family/caregiver and other team members in providing continuity of care.
  • Work in concert with the other hospice Interdisciplinary Team Members and community health related persons to coordinate the care plan.
  • Provide back-up for staff on-call RN as needed.

QUALIFICATIONS:

  • Graduate of an accredited school of nursing
  • Valid and Current NJ RN licensure.
  • Valid Driver’s license.
  • Excellent observation, verbal, written communication skills.
  • Excellent record of compliant EMR-based documentation.
  • Able and willing to drive to varied locations. At least three years of Hospice experience preferred.

Pay: $2,460.00 per month
