Responsibilities
- Conduct thorough patient assessments to evaluate emotional, social, and psychological needs within hospice care settings.
- Provide individual, family, and group counseling utilizing clinical techniques such as motivational interviewing, cognitive behavioral therapy, and psychotherapy to support coping strategies.
- Develop personalized care plans aligned with medical documentation standards and HIPAA regulations to ensure confidentiality and compliance.
- Collaborate with interdisciplinary teams—including medical staff, nurses, and therapists—to coordinate holistic patient care plans.
- Facilitate crisis intervention and manage complex cases involving mental health counseling, addiction counseling, or behavioral health challenges such as PTSD or dementia care.
- Collect and document data related to patient progress, treatment outcomes, and service utilization using Electronic Medical Record (EMR) systems.
- Assist with intake procedures for new patients, ensuring comprehensive diagnostic evaluation and appropriate referrals for specialized services like autism or developmental disabilities.
Qualifications
- Bachelor’s or Master’s degree in Social Work, Psychology, Counseling or related field; licensure or certification preferred.
- Proven experience working with hospice care, mental health counseling, or behavioral therapy in inpatient, outpatient, or home & community settings.
- Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
- Familiarity with crisis management techniques and working with individuals with disabilities or developmental disorders such as autism or Alzheimer’s disease.
- Experience conducting diagnostic evaluations and applying evidence-based practices including applied behavior analysis (ABA) and cognitive behavioral therapy (CBT).
- Ability to navigate EMR systems efficiently while maintaining strict adherence to HIPAA guidelines.
- Excellent communication skills for engaging with patients across diverse age groups—including pediatrics, geriatrics—and working in assisted living or nursing home environments. Pay: $75,000.00 - $80,000.00 per year
