Join our compassionate team as a Hospice Social Worker, where you will play a vital role in providing emotional support, counseling, and comprehensive case management to patients and their families facing life-limiting illnesses. You will collaborate closely with interdisciplinary teams to ensure patients receive personalized, respectful, and dignified end-of-life care.
Duties
- Conduct thorough patient assessments to evaluate social, emotional, and psychological needs in accordance with hospice care standards.
- Provide individual and group psychotherapy sessions utilizing techniques such as behavioral therapy, cognitive behavioral therapy, motivational interviewing, and clinical counseling.
- Offer crisis intervention and mental health counseling to support patients and families through complex emotional challenges.
- Coordinate with healthcare providers to develop and implement personalized care plans that incorporate social work interventions.
- Manage intake processes, document medical records accurately, and ensure compliance with HIPAA regulations for patient confidentiality.
- Facilitate communication between patients, families, and medical teams to promote understanding of diagnosis, treatment options, and available resources.
- Assist with discharge planning, home & community care arrangements, and connect families with additional support services such as assisted living or special education when appropriate.
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state law.
- Proven experience in hospice care, inpatient settings, nursing homes, or home & community care environments.
- Strong knowledge of medical terminology, ICD coding systems (ICD-9/10), CPT coding for documentation purposes, and medical records management.
- Familiarity with EMR systems for data collection and documentation; experience with HIPAA compliance is essential.
- Expertise in child & family counseling, behavioral health therapies including applied behavior analysis (ABA), addiction counseling, dementia care (including Alzheimer’s), PTSD care, and working with individuals with disabilities or developmental disabilities.
- Excellent skills in crisis management, diagnostic evaluation, case management, and working with diverse populations such as geriatrics and pediatrics.
- Ability to handle sensitive situations with empathy while maintaining professionalism; strong communication skills are vital for effective collaboration within multidisciplinary teams.
