Overview
We are seeking a compassionate and dedicated Hospice Social Worker to join our healthcare team. This role involves providing emotional support, counseling, and case management services to patients and their families during end-of-life care. The ideal candidate will have experience in hospice care, mental health counseling, and working with diverse patient populations, including children, seniors, and individuals with disabilities. The Hospice Social Worker will play a vital role in ensuring patients receive comprehensive psychosocial support aligned with their medical and personal needs while adhering to HIPAA regulations and maintaining accurate medical documentation.
Duties
- Conduct comprehensive patient assessments to identify psychosocial needs and develop individualized care plans.
- Provide clinical counseling services, including behavioral therapy, motivational interviewing, grief support, and crisis intervention.
- Assist patients and families with emotional support related to terminal illness, dementia, Alzheimer's disease, PTSD, or developmental disabilities.
- Coordinate with healthcare teams to facilitate patient care plans, discharge planning, and post-acute care arrangements.
- Manage intake processes and maintain detailed medical records using EMR systems in compliance with HIPAA standards.
- Offer group therapy sessions or individual counseling tailored to patient needs within hospice or community settings.
- Support families through difficult transitions by providing education on hospice services, advanced directives, and end-of-life planning.
- Engage in data collection for clinical research or quality improvement initiatives related to hospice care outcomes.
- Collaborate with other professionals in home & community care settings, assisted living facilities, nursing homes, inpatient units, or outpatient clinics.
- Ensure compliance with medical terminology standards such as ICD-10/ICD-9 coding for documentation purposes.
Experience
- Proven experience in hospice care or palliative settings is highly preferred.
- Background in mental health counseling, behavioral health, or social work with a focus on geriatrics, pediatrics, or special populations such as those with autism or developmental disabilities.
- Familiarity with medical documentation systems (EMR), CPT coding, ICD coding, and healthcare regulations including HIPAA.
- Experience working with individuals facing dementia, Alzheimer's disease, PTSD, or chronic illnesses such as cancer or terminal conditions is advantageous.
- Demonstrated ability to perform diagnostic evaluations and case management within outpatient or inpatient environments.
- Strong skills in crisis management, behavioral therapy techniques such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and motivational interviewing are desirable.
- Knowledge of home & community-based services and experience working in assisted living facilities or nursing homes will be considered a plus.
