Catholic Community Health is seeking a compassionate, experienced, and dedicated Hospice Social Worker to support patients and families throughout the Kansas City metro and surrounding communities.

This role is ideal for candidates with experience in hospice care, medical social work, palliative care, case management, discharge planning, counseling, grief support, and end-of-life care coordination.

As a nonprofit, faith-based hospice organization, our focus is providing compassionate end-of-life care, hospice services, palliative support, and holistic care that meets the physical, emotional, and spiritual needs of patients and families.

As a Hospice Social Worker, you will provide emotional support, psychosocial assessments, counseling, resource coordination, and guidance to patients and families during one of life’s most important transitions.

You will work closely with nurses, chaplains, physicians, caregivers, and the interdisciplinary care team to ensure patients receive compassionate, dignified, and individualized care.

Key Responsibilities

  • Complete psychosocial assessments for hospice patients and families
  • Provide emotional support, counseling, and crisis intervention
  • Assist with advance care planning and end-of-life decision support
  • Support families with grief counseling, bereavement resources, and coping strategies
  • Coordinate community resources and referrals for financial, emotional, and practical support
  • Collaborate with physicians, nurses, chaplains, and the interdisciplinary care team
  • Assist with discharge planning, placement needs, and transition support when appropriate
  • Maintain timely, accurate, and compliant documentation in MatrixCare EMR
  • Advocate for patient dignity, patient rights, and family-centered care

Qualifications

Preferred Experience

  • Experience in hospice, home health, palliative care, medical social work, hospital discharge planning, geriatric care, or healthcare case management preferred
  • Candidates with hospice, home care, palliative care, hospital social work, or end-of-life care experience are strongly encouraged to apply
  • Experience working with interdisciplinary healthcare teams is highly valued
  • Experience using MatrixCare EMR or healthcare documentation systems preferred

Required Education & Licensure

  • Master of Social Work (MSW) from an accredited program preferred
  • LMSW or LCSW licensure preferred based on state requirements
  • Bachelor’s degree in Social Work may be considered based on hospice experience and qualifications

Required Skills

  • Strong communication, counseling, and interpersonal skills
  • Compassionate, professional, and service-oriented mindset
  • Excellent organization, documentation, and time management
  • Ability to manage sensitive situations with empathy and professionalism
  • Strong understanding of grief support, crisis intervention, and family systems

Requirements

  • Valid driver’s license required
  • Reliable transportation for patient visits
  • Ability to pass background check and drug screening
