Responsibilities
- Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social support systems.
- Provide individual and group counseling sessions utilizing techniques such as behavioral therapy, motivational interviewing, and clinical counseling to address emotional distress and coping strategies.
- Collaborate with interdisciplinary teams to develop personalized care plans that incorporate hospice care principles, medical documentation, and compliance with HIPAA regulations.
- Assist patients and families with case management tasks, including resource coordination, advance directives, and end-of-life planning.
- Facilitate crisis intervention when urgent mental health or behavioral issues arise, ensuring immediate support and appropriate referrals.
- Maintain accurate medical records using Electronic Medical Record (EMR) systems while adhering to medical terminology standards like ICD-10 coding for documentation accuracy.
- Engage in community outreach and education about hospice services, dementia care, Alzheimer's care, PTSD care, and working with individuals with disabilities or developmental disorders.
Skills
- Proven experience in hospice care or home & community care environments with a strong understanding of patient care protocols.
- Expertise in child & family counseling, mental health counseling, behavioral health therapies such as cognitive behavioral therapy (CBT), and applied behavior analysis (ABA).
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for billing purposes, and familiarity with HIPAA regulations for patient privacy.
- Skilled in crisis management, intake procedures, diagnostic evaluation, and working with diverse populations including seniors, pediatrics, individuals with autism or developmental disabilities.
- Strong case management abilities with experience in outpatient settings or assisted living facilities; familiarity with inpatient or nursing home environments is a plus.
- Ability to utilize EMR systems effectively for data collection, documentation of medical records, and tracking patient progress.
- Excellent communication skills for conducting individual/group counseling sessions and facilitating family meetings related to hospice care planning.
