Job Summary
We are seeking a compassionate and experienced Hospice Social Worker to join our healthcare team. The ideal candidate will provide emotional support, case management, and counseling services to patients and their families facing serious illnesses. This role involves conducting patient assessments, coordinating care plans, and ensuring compliance with healthcare regulations such as HIPAA. The Hospice Social Worker will work closely with interdisciplinary teams to deliver comprehensive hospice care, including behavioral health, mental health counseling, and crisis intervention. Experience with diverse patient populations, including geriatrics, pediatrics, and individuals with disabilities or developmental disorders, is highly valued. Responsibilities
- Conduct comprehensive patient assessments to determine psychosocial needs and develop individualized care plans.
- Provide clinical counseling services such as individual, family, and group therapy focused on grief, coping strategies, and mental health support.
- Offer crisis intervention and behavioral health management for patients experiencing acute emotional distress or behavioral challenges.
- Coordinate with medical teams to ensure holistic patient care, including documentation of medical records and adherence to HIPAA regulations.
- Assist with intake procedures and facilitate communication between patients, families, and healthcare providers.
- Utilize EMR systems for accurate data collection, medical documentation, and case management activities.
- Support patients with Alzheimer’s disease, dementia, PTSD, autism spectrum disorders, or other disabilities through specialized counseling approaches like cognitive behavioral therapy or applied behavior analysis.
- Engage in clinical research efforts related to hospice care practices or behavioral health interventions when applicable.
- Participate in community outreach or home & community care programs to enhance patient support outside clinical settings.
Experience
- Proven experience in hospice care or palliative settings is preferred.
- Background in child & family counseling, addiction counseling, or behavioral therapy is advantageous.
- Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical records management is desirable.
- Experience working with individuals with developmental disabilities or in special education settings is a plus.
- Prior involvement in inpatient, outpatient, nursing home, assisted living facilities, or post-acute care environments is beneficial.
- Skills in crisis management, diagnostic evaluation, case management, motivational interviewing, and mental health counseling are essential.
- Knowledge of EMR systems and experience working within HIPAA guidelines are required for effective documentation and patient confidentiality.
