Responsibilities

• Conduct comprehensive patient assessments to evaluate social, emotional, and environmental needs within hospice settings.
• Develop individualized care plans that incorporate end-of-life care principles, behavioral therapy techniques, and community resource linkage.
• Provide direct social work services including crisis intervention, mental health counseling, grief support, and family education.
• Collaborate with medical professionals to facilitate discharge planning, medical documentation, and coordination of hospice services.
• Assist patients and families with navigating healthcare systems such as Medicare and Medicaid while ensuring compliance with HIPAA and clinical ethical guidelines.
• Collect data for clinical research initiatives related to hospice care outcomes and behavioral health interventions.
• Maintain accurate medical records using EMR (Electronic Medical Records) systems while adhering to documentation standards.

Qualifications

• Bachelor's or Master’s degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state law.
• Proven experience working with geriatric patients, individuals with disabilities, dementia care (including Alzheimer’s), or those requiring end-of-life support.
• Knowledge of hospice & palliative medicine practices, medical terminology, ICD coding, and discharge planning procedures.
• Demonstrated skills in child & family counseling, behavioral therapy techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and crisis management.
• Familiarity with community resource coordination for senior care, assisted living facilities, home & community care experience, and inpatient/outpatient settings.
• Ability to work effectively with diverse cultural backgrounds and individuals with developmental disabilities or autism spectrum disorder (ASD).
• Strong communication skills for client interaction, group therapy facilitation, case management, and clinical interprofessional collaboration.
• Competency in medical documentation standards including HIPAA compliance and clinical ethical guidelines; proficiency with EMR systems is preferred.

Pay: $35.00 - $45.00 per hour
