Location
Elizabeth House Hospice Facility in Flat Rock, NC
Provides excellent care that follows physician and hospice/palliative care procedures/requirements, practicing within the scope of license, certification or registration, where applicable, and as required by law and regulation.

Works to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies

Provides direct client care, as attending or consulting physician, to hospice/palliative clients in a hospital, hospice facility, nursing homes, assisted living facilities, and/or client homes
-
Provides medical aspects of care in collaboration with the interdisciplinary team of attending physicians, nurses, social workers, certified nursing assistants, and chaplains

Actively participates in weekly Interdisciplinary Team meetings for development and evaluation of the client’s plan of care
-
Participates in the coordination of the patient’s hospice care in accordance with professional standards of practice

Participates in the development and evaluation of the plan of care
-
Contributes to patient and family counseling and education

Performs duties in compliance with Medicare Condition of Participation

Assesses patients for participation in research studies

Maintains relationships with community physicians
-
Acts as the designee of the Hospice Medical Director as required based on the needs of the agency.

Knowledge of HIPAA privacy rules and regulations

Knowledge of electronic records systems

Proficient in MS Office: Outlook, Word, Excel, etc.

Education:
- Doctor of Medicine degree or Doctor of Osteopathic Medicine degree from an accredited institution
Experience:
-
1-3 years of applicable experience in the treatment of end of life care

Demonstrated understanding of the Centers for Medicaid and Medicare Services requirements

Experience effectively collaborating with a diverse group
Licensures & Certifications: (if applicable)

Possess and maintain any and all applicable privileges with Mission, Pardee and/or Park Ridge hospitals
-
Valid, unrestricted, medical license to practice medicine in the state of North Carolina
-
Board Eligible or Board Certified in Hospice and Palliative Medicine, preferred
-
Valid, unrestricted, DEA registration
-
Valid driver’s license and proof of automobile insurance
