Hospice Weekend RN

*In Person Visits Required - Not a remote position*

Radiant Hospice seeks a compassionate RN experienced with providing direct in-home care to patients approaching end of life. Hospice RN serves as backup assist to the weekend team to assist with troubleshooting and completes referral management on the weekends. The Hospice RN visit patients as needed and coordinates with the IDG team to ensure continuity of care for the hospice patient.

Pay Structure:

• Hourly rate paid for meetings, office, and training time
• Weekend standby rate ($400) paid per available weekend (Friday 5pm-Monday 8am)
• Per Visit rate ($125) paid per completed Patient Visit (Routine)
• Per Visit rate ($250) paid per completed Patient Admission
• Monthly Technology Stipend
• Mileage Reimbursement

The Hospice RN oversees the hospice patient's plan of care care and will collaborate with the other members of the patient's care team, attending physician, and patients and their families to coordinate quality care. The Hospice RN evaluates a patient's physical condition and develop care plans that address any of the patient's medical needs or needs for social and spiritual services. Apart from providing comfort to patients, The Hospice RN is adept at counseling and guiding patient's families as well.

This position is offered with mileage reimbursement, technology allowances, competitive rates and bonus opportunities. Minimum Qualifications:

• Graduate of an accredited nursing program.
• Current California Registered Nurse license.
• At least one (1) year of hospice or palliative care experience
• Minimum of two (2) years of experience as a professional nurse within the last five (5) years.
• Experience in a case management role
• Expertise in pain management, symptom control, crisis intervention, and family support.
• Excellent demonstrated critical thinking skills and superior clinical judgment.
• Excellent computer skills.
• Excellent customer service skills.
• Reliable cell phone with local number and coverage.
• Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable personal transportation.
• Ability to work independently and manage time effectively. Ability to make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting.
• Acceptance of and adaptability to different social, racial, cultural, and religious modes.
