The Role: What You Will Do

The Hospital Liaison is a critical, highly visible role responsible for developing and maintaining strong working relationships with local hospitals, physicians, and discharge planners. You will be the primary link between the hospital and our facility, ensuring a smooth, compliant, and timely transition for patients referred to our skilled nursing and rehabilitation services.

  • Relationship Management: Serve as the primary point of contact for hospital case managers, social workers, and physicians to maintain and grow referral relationships.
  • Patient Assessment: Conduct on-site medical record reviews and financial assessments of potential admissions to ensure patients meet the criteria for SNF level of care.
  • Admissions Coordination: Facilitate the entire admission process, including managing all necessary paperwork and effectively communicating patient needs to the SNF's interdisciplinary team.
  • Marketing & Education: Act as an informed representative of our facility, educating hospital staff on our clinical capabilities, services, and admission criteria.
  • Market Intelligence: Track hospital discharge trends and competitor activity to inform the facility's strategic planning.

Qualifications: What You Will Bring

Required

  • Education: Bachelor's degree preferred, or equivalent experience in a related healthcare field (e.g., healthcare administration, marketing, social work).
  • Experience: Minimum of 2-3 years of experience in a healthcare setting, with at least 1 year in a hospital liaison, admissions, case management, or high-volume referral management role.
  • Clinical Acumen: Demonstrated ability to understand and interpret patient medical charts, diagnoses, and discharge planning needs.
  • Knowledge: Comprehensive understanding of Medicare, Medicaid, managed care, and basic insurance verification/authorization processes related to skilled nursing care.
  • Skills: Exceptional communication, organizational, and presentation skills. Must be a goal-oriented, self-motivated professional.
  • Other: Valid driver's license and reliable transportation for frequent travel between the SNF and local hospitals.

Preferred

  • Experience: Experience working within an acute care, rehabilitation, or skilled nursing facility setting.
  • Local Network: Established professional relationships with hospital staff and healthcare providers in the Zion area.

Benefits and Compensation

  • Compensation: $65,000-$80,000 depending on experience and qualifications
