Responsibilities

• Conduct thorough physical examinations and patient assessments to determine diagnosis and treatment plans.
• Manage acute medical care cases in General medical floors, transitional care unit and Acute rehab floor.
• Collaborate with physicians, nurses, therapists, and case managers to coordinate comprehensive patient care and discharge planning and attend daily Multidisciplinary rounds.
• Utilize Epic to document medical records accurately and efficiently.
• Ensure compliance with HIPAA regulations and while maintaining high-quality patient care standards.

Experience

• Proven experience working in hospital medicine preferred.
• Demonstrated proficiency with EMR/EHR systems such as Epic.
• Strong background in managing Hospital medicine patients.

Pay: $116,176.37 - $139,911.31 per year
