Job Summary
We are seeking a compassionate hourly Certified Home Health Aid to join our team in providing exceptional in-home care for individuals with diverse needs.. Monday - Friday

8-hour shifts - flexible times.

MUST drive and have your own vehicle.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility assistance using tools like Hoyer lifts when necessary.
• Monitor and observe patient conditions, including vital signs, medication adherence, and overall health status, documenting findings accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
• Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques.
• Support clients with assisted living needs by helping with meal preparation, light housekeeping, laundry, and maintaining a clean and safe environment.
• Communicate effectively with healthcare teams regarding patient progress and document medical records thoroughly to ensure continuity of care.

Qualifications

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
• Proven experience working with individuals with disabilities or in long-term care settings such as nursing homes or assisted living facilities.
• Knowledge of medical terminology, CPR certification, and medication administration protocols.
• Experience working with EMR/EHR systems for accurate documentation of patient information.
• Familiarity with dementia care, Alzheimer's care, hospice support, catheter care, and memory care is highly desirable.
