House Coordinator w/ coverage rotation
Chances of Hope LLC is seeking a House Coordinator that works in the home directly with individuals with disabilities and is responsible for professional planning, management, and supervisor of community residential services in various sizes serving individuals with who are prone to exhibiting impulsive behaviors, poor decision making, and verbal/physical aggression. This person functions as the Degreed Professional Manager (DPM) (previously the Qualified Developmentally Disabled Professional (QDDP)) with the overall responsibility for the development, the implementation, the coordination and the evaluation of the individual personal plans adapted to the needs of the individuals. This person will be responsible for developing and monitoring individualized personal plans, supervising assigned staff, preparing work schedules, ensuring staff coverage, reporting and recording observations, circulating and implementing directives and maintaining records and reports. This person will be expected to exercise independent judgement and initiative within the limits of established guidelines and procedures. You will be required to attend all planning meetings and transporting individuals when necessary. You will be required to work the coverage rotation, working open shifts as needed. This is a Full-Time, salary position.
Requirements:
Graduation from an accredited college or university with a Bachelor’s Degree (major specialization in elementary or secondary education, special education, early childhood education, psychology, social work, sociology, counseling, recreation, speech language pathology, or audiology, occupational therapy, physical therapy, nursing, or other specialties in the field of human services) AND one year of experience with individuals with developmental disabilities is preferred.

5 years of relevant experience may substitute for a degree.
Required Certifications:
Prior certification in First Aid and CPR is preferred, but training can be provided prior to the start date.
Prior certification in MANDT/CPI is preferred, but training can be provided prior to the start date.
Prior certification in Medication Administration is preferred, but training can be provided prior to the start date.
Successfully complete a course in Abuse and Neglect.
Must have valid Class E driver's license or able to obtain one.
Must have up to date car insurance.
Must have open availability.
